Karen begins the episode as a "Computer Fairy" telling the episode's story. At the Krusty Krab, Plankton notices Patrick eating a huge pile of Krabby Patties, but he can't decide on which patty to steal. Patrick crushes him and assumes he's hearing a "crazy voice who keeps telling me what to do." SpongeBob explains that it's his conscience, "a tiny voice inside you that tells you the difference between something that's right and something that's wrong."

Plankton decides to play along and tells Patrick that he is his "conscience." Patrick agrees to buy some Krabby Patties for Plankton "right after I run some errands." Plankton is against Patrick running "errands" but agrees to let him do so on the condition that he runs his "errands" quickly.

At Goofy Goober's, Patrick high-fives everyone at the restaurant (even Plankton) before asking for a chocolate molar blowout. Plankton tells Patrick that he hates ice cream and he only wants Krabby Patties, and Patrick asks if his nose grows when he tells lies, but Plankton states that "Noses only grow when you make up the truth." Patrick does so, and he and Plankton almost cause a "nose fight." Patrick then finds that he only has one errand left: "Gotta give Plankton a Krabby Patty."

Plankton demands that Patrick toss a Krabby Patty into the slot of a street mailbox. However, Pearl and her friends arrive, heading to Goo Lagoon. The patty is stolen by Pearl, and Patrick pursues the patty on Plankton's orders. When they arrive, Pearl has already eaten the patty and is sleeping. Patrick goes into Pearl's mouth to retrieve the patty.

Inside Pearl's mouth, Patrick and Plankton meet Geppetto, an Italian fish who has been living inside Pearl. Patrick takes a wig while Plankton steals the patty, and Geppetto tells his son, Pinocchio, to give chase. At Goo Lagoon, Pearl is suffering from indigestion.

Karen the Computer Fairy arrives and, in infomercial fashion, gives Pearl "Gush and Flush," a indigestion-relief product. Pearl tries the Gush and Flush, and everything she ate comes out of her. Plankton tries to leave with the patty, but Patrick's real conscience, which is dumb as the real deal, identifies Plankton as a bug and tells Patrick to step on him, which he does. Karen the Computer Fairy signs off as the episode ends.
