Seen in the episode SB-129 when Squidward went back in time. Prehistoric Patrick is Primate Sponge's friend. But they're not smart since they have no common sence. Prehistoric Patrick and Primate Sponge's language is called jibjab.
Looks
Prehistoric Patrick has big teeth, a loin cloth,and a big mouth. He's pink like Patrick but looks quite different from him.
